Job Description:

* Hawthorn East location
* Friendly and supportive team environment
* Permanent, part time, (24 hours per week over 7 days)

About the Role
You are an experienced Mental Health Clinician who is passionate about providing high quality mental health treatment and care, for consumers who present with complex mental health conditions. You will be providing outreach to consumers in their homes and within clinics, providing recovery orientated, strengths-based care.

* Assessment (inclusive of risk assessment and management, bio-psychosocial), discipline specific interventions and case management
* Monitoring of MSE and medication compliance
* Completing Outcome measures and all related requirements for Activity based funding
* Implement integrated care for consumers who present with broad substance use

Classifications for this position will range from

* RPN Grade 3 NP81 - NP74 ($100,932 - $112,091 per annum)
* OT/SW Grade 2 YB20 - YB23 or YC42 - YC45 ($88,030 - $101,19720 per annum)

Your Contribution

* A minimum of at least 2 years post qualification clinical experience within Mental Health for SW, OT and RPN
* Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work)
* RPNs - Nursing and Midwifery Board of Australia (NMBA)
* Relevant postgraduate qualification in mental health for nursing staff
* Occupational Therapists - Registration with the Occupational Therapy Board of Australia
* Psychologists - Registration as a psychologist under the Psychology Board of Australia. Practice endorsement as a Clinical Psychologist or eligibility to enter a Clinical Psychology registrar program with the Psychology Board of Australia; or
* Social Workers - An approved degree in Social Work (by the AASW) and eligibility for full membership of the Australian Association of Social Workers. International Social Workers will need to be able to provide evidence of their assessment of International qualifications from the AASW (inclusive of the reference number).
* Current Victorian driver's license
* A valid Nationally Coordinated Criminal History Check, or willing to obtain
