About the Role
We are seeking a compassionate individual to join our team as a Care Partner. As a key member of our Community Care branch, you will play a vital role in supporting our older generation in living independently.
As a Care Partner, you will be responsible for planning, managing and leading client care and support across a range of programs. You will also coordinate the involvement of our multi-disciplinary teams and other services to ensure effective care.
* Manage and coordinate care services to deliver the highest standards of care.
* Develop and implement tailored care plans that cater to individual client needs.
* Liaise with clients, families and healthcare professionals to ensure comprehensive care is provided.
* Foster strong relationships with clients, stakeholders and team members, while assisting clients in meeting their unique needs and goals.
* Ensure compliance with all relevant legislation and organizational policies.
* Monitor and evaluate care services to continuously improve quality and efficiency.

Requirements
To succeed in this role, you will need to demonstrate experience in case management and client care coordination in a Community Aged Care setting. You must have strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people. Additionally, you will need to be vaccinated against Influenza and at least one dose of Covid-19 vaccine. Proof of immunity or willingness to become vaccinated against Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) and Pertussis (Whooping Cough) is also required.
