Key Responsibilities:
The position of Clinical Care Coordinator entails providing comprehensive care and coordination to facilitate recovery and wellbeing for clients of the Prison Mental Health Service.
This includes developing and implementing individualized treatment plans in collaboration with multidisciplinary teams, ensuring a holistic approach to client care.

Additionally, this role involves collaborating with relevant stakeholders within the correctional environment and community, facilitating communication and ensuring effective service delivery.
Key qualifications include occupational therapy, psychology, social work, and registered nurse. The ideal candidate will possess sound knowledge of mental health services, principles, and practices, as well as relevant laws and regulations.
