Aboriginal Child & Youth Health & Wellbeing Navigator

Do you want to make a real difference in the lives of Aboriginal children and young people?

We are seeking a dedicated and culturally respectful person to work alongside Aboriginal children and young people living in Out of Home Care. In this community-focused role, you will provide practical, emotional, and cultural support to help children and young people feel safe, connected, and empowered in their daily lives. You will build strong, trusting relationships with young people, carers, families, and community, supporting cultural identity, wellbeing, and connection to Country.

This role is ideal for someone who is passionate about walking alongside Aboriginal children and youth, advocating for their needs, and creating positive, strength-based outcomes in a nurturing and supportive environment.


Responsibilities

* Support Aboriginal children and young people in out of home care to get the health care and wellbeing support they need
* Help organise health checks, screenings, and appointments in a culturally safe way
* Work with children, families, and carers to create and review care plans that feel right for them
* Work closely with Child Protection, carers, and other service providers


Supporting families

* Assisting families to sign up for Medicare and Centrelink
* Supporting families to enrol children in childcare or preschool, including helping with forms and visiting centres
* Helping families understand and apply for funded preschool programs
* Supporting parents and carers during the transition to parenthood
* Connecting families to playgroups, baby groups, and women's groups in the community


Requirements

* A relevant qualification and registration (RN/EN, Social Worker, Aboriginal Health Practitioner or similar)
* An understanding of clinical and holistic careservices and processes.
* The ability to develop client-centred care plans and coordinate service pathways
* Strong relationship-building, advocacy and communication skills
* Cultural awareness and a strengths-based approach to care
