Ozcare is currently seeking a dedicated professional to join our team as a Care Partnering Lead. This role will be responsible for providing leadership to a Care Manager team in the planning, managing and leading client care and support across various programs.
This position requires strong leadership skills, excellent communication abilities, and a passion for delivering high-quality care services. The successful candidate will have experience in case management, client care coordination, and staff development.
Key Responsibilities:
* Manage and coordinate care services to ensure the highest standards of care.
* Develop and implement care plans tailored to individual client needs.
* Liaise with clients, families, and healthcare professionals to ensure comprehensive care.
* Foster strong relationships with clients, stakeholders, and team members.
* Ensure compliance with all relevant legislation and organizational policies.
Requirements:
* Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
* Strong verbal, written, and interpersonal communication skills.
* Ability to develop staff, foster teamwork, and work effectively as a team member.
* Provide proof of immunity against Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough).
* Current First Aid and CPR Certificate.
* NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check).
* Current National Police Check.
* Hold a current Queensland driver's licence.
* Relevant tertiary qualifications and/or experience in a health-related field.
