Right at Home, Adelaide Region are seeking qualified, reliable, and passionate Care Coordinator ( Registered Nurse). You will need to be friendly, positive, and determined to make a difference. The Ideal Candidate: The ideal candidate for our team will have at least 2 years experience with managing client enquiries, client consultations, client onboarding, and a client caseload for Home Care Package, NDIS, and private pay clients. The care coordinator will have experience managing the complex scheduling requirements for community care workers.

Purpose of Position: This position places a strong emphasis on prioritizing customer satisfaction and involves engaging with families, conducting in-home care assessments, and formulating customized care plans tailored to meet the specific needs of our clients. The role also encompasses supporting administrative personnel in various aspects such as recruitment, training, and overseeing the performance of our caregivers and nurses.

Key Responsibilities:

Provide compassionate and client-centred support by responding to a high volume of phone and online enquiries, ensuring timely and empathetic communication.

Meet with clients to understand their unique needs, complete in-home care assessments, and develop tailored care plans aligned with Aged Care Quality Standards.

Address a wide range of day-to-day client matters, including general enquiries, feedback, service adjustments, and complaints, with professionalism and a solution-focused approach.

Support the Rostering team by assisting in the creation of master rosters and performing scheduling tasks as required to ensure efficient service delivery.

Manage daily scheduling changes and ensure care is delivered according to client needs and staff availability.

Support the care team through training, mentoring, and performance monitoring to ensure high-quality, person-centred care.

Conduct staff assessments and assist in credentialing care workers for medication administration, fostering a skilled and confident team.

Maintain accurate and compliant documentation for both clients and staff, in line with government regulations and organisational policies.

Demonstrate adaptability and a proactive mindset by embracing change and taking challenges as opportunities for growth.

Participate in a rotating “on-call” system to ensure clients and staff receive timely support after hours, on weekends, and public holidays.


Qualifications

QUALIFICATIONS:

Possession of current AHPRA registration (RN) (specialized in Ageing or Disability & Community )

Current Australian (Satisfactory) National Police check, Working with Children Check,

Current First Aid & CPR certificate.

Driver's license and your own reliable registered vehicle

Excellent verbal and written communication skills

Professional Indemnity Insurance (RN)

A smart mobile phone (iPhone / android)


Preferred Skills

Preferred Skills :

At least 2 year experience as a scheduler, case manager, care coordinator ( Home Care Package ) and hospital discharge planner

Solid knowledge in the Disability and/or Aged Care sector(s)

Must be Registered Nurse .

The ability to operate efficiently and deliver excellent service when under pressure.

Clinical skill in wound care

Strong time management and organizational skills to manage competing priorities effectively.

Relationship development skills to support diverse clients to achieve their goals and objectives.

Familiar with Visual Care

Intermediate to Advanced Microsoft Office experience (Excel, Word, Outlook, and CMS)
