We are seeking dedicated Pharmacy Technicians to work as part of a multidisciplinary team, providing clinical support and advice to patients focusing on best practice and adhering to safety guidelines.


Main responsibilities

As a remote Pharmacy Technician, you will confidently act within professional standards and boundaries, supporting and working alongside your General Practice team as well as Pharmacists and Pharmacy Technicians. You will help resolve medication-related queries, improve patient outcomes, and support the improvement of access to healthcare services.

* Medication Reconciliations using clinical letters and post Hospital Discharge Summaries for enhanced patient care.
* Medicine Management including Prescription screening and processing with attention to detail.
* Respond and communicate effectively to patient related medication queries in a timely manner.
* Communicate with local pharmacies regarding stock supplies issues and shortages for streamlined operations.
* Medication synchronisation in line with regular repeat prescription schedules for patient convenience.
* Medication monitoring recalls (e.g BP, blood tests, annual reviews) to ensure optimal health outcomes.
* Prescribing audits in line with CQC/QOF targets as set by the Prescribing Team for quality assurance.
* Standard/Annual medication monitoring to identify potential risks and prevent adverse reactions.
* Shared Care Protocols for collaborative patient care with multiple healthcare providers.
* Action medicines safety alerts from CAS/MHRA/ICB to stay updated on the latest regulations.
* Level 1 and 2 Medication reviews to provide comprehensive patient assessment.
* Assist with Pre-SMR (Structure Medication Review) Screening to facilitate smooth patient transitions.


Requirements

To be successful in this role, you will need:

* A strong understanding of pharmacy practices and procedures.
* The ability to work independently and as part of a team.
* Excellent communication and interpersonal skills.
* A commitment to maintaining confidentiality and handling sensitive information.
* Able to adapt to changing situations and priorities.
* Familiarity with relevant laws and regulations governing pharmacy practice.
