Croydon Community Mental Health Service (CMHT) has undergone service transformation to enhance the quality of our service delivery, improve the experiences of those using our services, maintain staff morale and ensure services meet the NHS Long Term Plan. The Community Mental Health Team (CMHT) is a generic team integrated with Croydon Social Care, working with all adults experiencing severe and enduring mental illness.
The teams are no longer based on diagnosis and work with individuals from a locality who have mental health needs that require secondary care support. Croydon CMHT has three localities: NORTH, CENTRAL and SOUTH. Our Locality Teams are highly experienced, supportive and multidisciplinary. We are looking for enthusiastic care coordinators who will grow with the locality teams from Nursing, Social Work and OT professional backgrounds.

Main duties of the job
In line with the new Coronavirus Regulations 2021 regarding Care Home visits, the successful candidate will need to provide evidence of full vaccination status against Covid‐19. The post holder will work as Care Coordinator / Community Mental Health Nurse for a caseload of service users in the local community with complex mental health difficulties and will collaborate with their carers where applicable. Expected skills include excellent self‐management, collaborative teamwork, flexible, robust and holistic care planning with service users and/or carers, ability to work well under pressure, experience in dynamic risk assessment, and outstanding communication skills.

Person Specification
Qualifications
* Relevant professional qualification – Mental Health Nursing, Social Worker, Occupational Therapist with affiliated professional registration (NMC, HCPC).
Experience
* Experience of interagency working.
* Experience of managing a caseload.
* Significant relevant clinical experience.
* Experience of working with diverse groups of patients and staff.
* Experience of working in the community.
* Experience supporting practice development in clinical areas.
Skills and Abilities
* Knowledge of CPA, Community Care Act, Carers Act, Mental Health Act and other relevant legislation as applied to practice.
* Skills in mental health and risk assessments.
* Understanding of Care Coordination and CPA.
* Responsive to service users' and carers' needs.
* Understanding of recovery in mental health.
* Strong grasp of holistic care plans.
* Problem‐solving, ability to identify issues, review options and make decisions.
* Skill in supervising others and providing constructive feedback.
* Knowledge of clinical governance and audit.

* Empathy for service users, including those who have experienced mental health problems.
