Hospital Liaison General Practitioners

Location | John Hunter Hospital Campus New Lambton Heights

Hours | Part-time (8 hours per week) until 30 March 2026

Hourly rate | $155 per hour + Superannuation

WhyThese two roles have been created to lead an innovative pilot program that will transform how hospital and primary care services work together in the Hunter New England region. The Hospital Liaison General Practitioner (HLGP) service is the first of its kind locally, designed to create connections between care settings, ensuring people experience smooth transitions after discharge from hospital.

The two HLGPs will work with a project officer and play a pivotal role in shaping this new model of care in partnership with HNELHD, HNECC PHN, and HMRI.

About the roleThis is an exciting opportunity to be part of a groundbreaking initiative – the first Hospital Liaison General Practitioner (HLGP) service in the Hunter New England region. This newly created, non-patient facing role is central to a pilot program designed to bridge the gap between hospital and primary care, improving patient experience, health outcomes, and clinician collaboration.

As the HLGP, you'll bring your primary care expertise and leadership to support a 'one system' approach. You'll act as a trusted resource for GPs, help navigate services for recently discharged patients, and work closely with hospital teams to identify and address service gaps. This role offers the chance to shape integrated care models, influence system improvements, and contribute to research and evaluation alongside partners including Hunter New England Local Health District (HNELHD), Hunter New England and Central Coast Primary Health Network (HNECC PHN), and Hunter Medical Research Institute (HMRI).

The successful candidate will have:

* Minimum 5 years' experience working as a GP post Fellowship
* A current role as a GP working within the Hunter New England area
* Strong ability to build relationships with stakeholders to support integrated patient care.
* Ability to act as a trusted contact for GPs, assisting with service navigation and care coordination.
* Skills in identifying service gaps and contributing to solutions that reduce unnecessary hospital visits.
* Excellent communication and collaboration skills across multidisciplinary teams.
* Commitment to clinical governance, confidentiality, and organisational policies.
* Ability to provide primary care advice to leadership and contribute to evaluation activities.
* Competence in data collection and reporting for research and service improvement.
* Proactive approach to problem-solving and participation in team and external meetings.
