Belong at Home (CHC) is seeking an enthusiastic, motivated and caring person to join our team as a Client Services Coordinator (Care Partner).
This is a fulltime position with a highly respected not-for-profit community organisation providing care and support services across the South West of WA.
We operate throughout the South West with branches in Bunbury, Busselton, Margaret River, Collie, Donnybrook and Bridgetown.
In this rewarding role you will work in a team environment with a can do attitude where you will be responsible for a portfolio of clients of both CHSP and HCP and provide effective support to them by developing, reviewing and managing their client centred care plans and individual budgets.
The fulltime role is based in Busselton with some travel required throughout the surrounding area to meet business requirements.
As a Client Services Coordinator (Care Partner) you will:
* Process client referrals and service requests including point of contact for referral enquiries within specified timeframes.
* Assess and collaborate with clients, their representatives and community supports to plan and achieve the best outcomes for possible for clients so they can remain independent and connected in their community.
* Manage client information on the My Aged Care Portal (MAC)
* Establish and maintain client records on a Sandwai database
* Liaise with Schedulers to ensure scheduling services are completed
* Work collaboratively with team members on compliance ensuring the highest standards are met.
* Other duties in alignment with qualifications and experience.
Essential:
* A good working knowledge of the services provided in the aged care industry
* Customer service focussed with a commitment to meeting the needs of our clients.
* Minimum 2 years experience in a similar position.
* Preferred Certificate 3 or higher in aged or community care, Diploma of Community Services or relevant qualifications in Allied Health, Nursing, or related field.
* Excellent verbal and written communication skills.
* A genuine enthusiasm, care and compassion in your commitment to working with the elderly.
* A current "C" class WA Driver's License is essential
