We're currently seeking a passionate and collaborative team player to join our Connecting Home service, supporting individuals who are experiencing or at risk of homelessness—specifically focusing on mature-aged participants—across Lismore and Grafton areas.
You will play a key role in helping participants access safe, sustainable housing solutions and connect with the supports they need. No two days will be the same—whether you're offering phone-based support, conducting outreach, or working alongside other community-based services to deliver strength based, person-centred care.
Job Description
This is a fantastic opportunity for someone who:
- Brings experience or transferable skills in case management, community services, or housing support
- Thrives in a collaborative environment and enjoys building strong relationships with participants, service providers, and stakeholders
- Is proficient in conducting needs and risk assessments with care and professionalism
