Case Manager

About the Role
The successful candidate will be responsible for managing a client case load and participating in the delivery and planning of high-quality care and support services.
* Conduct comprehensive initial assessments and develop treatment plans that meet individual needs.
* Provide advocacy and support for clients who require assistance from external agencies.
* Develop and maintain sound working relationships with key stakeholders.
Key Responsibilities
The Case Manager will:
* Manage a caseload of clients and provide individualized support and guidance.
* Work closely with other staff members to ensure the delivery of high-quality care and support services.
* Identify opportunities for improvement and implement changes as needed.
Required Skills and Qualifications
To be successful in this role, you will need:
* Cert IV in Community Services or Social Emotional Wellbeing (desirable).
* Previous experience in case management, preferably within a similar service.
* A current Open QLD driver's license.
* A current First Aid and CPR certificate or willingness to obtain.
