**Team Leader – Diabetes Co-management Service, Hospital Admission Risk Program (HARP)

About The Royal Melbourne Hospital (RMH)
RMH part of the Parkville Precinct and is one of Australia's leading public hospitals, providing a comprehensive range of specialist medical, surgical, and mental health services, as well as rehabilitation, aged care, specialist clinics and community programs.
HARP provides specialist treatment, care planning, education and support to help people with chronic and complex health issues to manage independently and reduce their risk of a hospital admission. The Diabetes Co-Management Service operates as part of the Hospital Admission Risk Program (HARP). The service is available to individuals with unstable diabetes who have been admitted to hospital or presented to the emergency department (ED) during the last 12 months.
The role of the Diabetes Co-Management Service Team Leader is to provide leadership, professional and operational oversight for the service. The role ensures effective human resource management including supervision, recruitment and performance management and has a role in providing direct clinical care.
Your New Role
* Work closely with a fabulous and highly experienced multidisciplinary team to provide Diabetes management to people with complex needs
* Work collaboratively with consumers, their families, general practitioners and specialists at The Royal Melbourne Hospital
* Contribute to the ongoing development of the team, service and HARP program
* Work in a leadership role while continuing to practice clinically across hospital, community and home-based care settings
About You
* You are registered as a Division 1 Registered Nurse under the Australian Health Practitioner Regulation National Law Act with the Nursing and Midwifery Board of Australia and
* You have completed the Post Graduate Diabetes Educators Course accredited by Australian Diabetes Association (ADEA); and are an ADEA Credentialed Diabetes Educator;
* You are passionate about the management of diabetes including in people with complex psychosocial circumstances
* You have experience working in hospital and/or community settings and understand the factors contributing to hospital admission in clients with complex health issues
* You have previous experience leading and supervising teams
* You are flexible, energetic, innovative, and can work cooperatively in a multidisciplinary team as well as autonomously
* As of January 1st you must hold a valid employee Victorian Working With Children's Check
