The Care Coordinator will be responsible for conducting comprehensive client assessments and providing professional health and clinical care advice to home care workers.
Key Responsibilities:
* Developing meaningful relationships with clients and their families through regular communication
* Conducting thorough client assessments to identify needs and develop care plans
* Providing expert health and clinical care advice to support the delivery of high-quality home care services
* Collaborating with clients and their families to ensure their needs are met
* Maintaining accurate and up-to-date client records
Requirements:
* Formal tertiary qualification in nursing or a related field
* Registration as a Registered Nurse with AHPRA
* Proven knowledge and experience within the aged care sector
* National police check and industry applicable licenses
* Valid First Aid and CPR certificate
* Minimum of 2 COVID vaccinations
