Job Role:
Clinical or Non-Clinical Care Partner
1. Carry out comprehensive in-home assessments to develop tailored care plans.
2. Manage participant cases, providing expert clinical and holistic support.
3. Track and document participants' wellbeing, progress, and goals.
4. Maintain close collaboration with medical professionals, allied health specialists, and service providers.
5. Ensure compliance by maintaining accurate documentation (CRM, funding, evidence uploads).
6. Oversee participant budgets and ensure adherence to funding guidelines.
7. Provide training and guidance to support workers and nurses, promoting their professional growth.
8. Schedule and coordinate services, including urgent changes, to meet participants' needs.
9. Contribute to ongoing audits, reporting, and professional development initiatives.
10. (Clinical Care Partners only) Provide occasional wound care services.
Key Qualifications:
Registration/enrollment as a nurse is required. Additional qualifications may be necessary depending on the specific role.
