About the Role

Join our team as a Grade 3 Social Worker, on a part time (0.8 EFT), ongoing basis. You will provide a clinical NDIS lead role to support our acute and emergency departments across Monash Medical Centre, Dandenong Hospital and Casey Hospital.

As the Grade 3 Social Worker, you will have consolidated psychosocial assessment and discharge planning skills and use evidence to inform practice to enable you to carry a clinical caseload and provide a specialist NDIS knowledge base to support and provide consulation to our social work clinicians in the acute and emergency department settings.

Having a sound understanding, and working knowledge of the range of the service systems including Family Violence, Child Protection, My Aged Care, Transition Care Program, with a specialist knowledge base in the National Disability Insurance Scheme (NDIS), to ensure we provide high quality, evidence based social work service to all patients and families at Monash Health. It will also be expected that you will have responsibilities and actively participate in quality and research activities. You will work in accordance with and be professionally accountable to the standards of the AASW and Social Work Credentialing and Scope of Practice, Supervision, Code of Ethics and Practice Standards.

About Us

The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists. The Social Work Service operating hours are standard business hours Monday to Friday. A weekend service is provided by roster at Casey, Clayton, Dandenong and Victorian Heart Hospital sites and an on-call service is also provided to the acute sites.

About You

You are a motivated and skilled social worker, ready to take on new challenges and further develop your clinical and leadership capabilities. You bring experience in delivering specialised clinical services and are confident in conducting complex psychosocial assessments within a dynamic caseload.

You thrive in a collaborative, multidisciplinary environment and are committed to providing person-centred, evidence-based care across both inpatient and community settings. Your strong communication skills and ability to build effective relationships with internal and external stakeholders make you a valued team member.

You're eager to expand your impact through leadership opportunities, involvement in research and project work, and by contributing to ongoing service development within a supportive Social Work and Allied Health team.

What you need:

* Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
* Specialist knowledge of the National Disability Insurance Scheme
* Eligible for ordinary membership of AASW if overseas trained )
* A current Victorian Driver's Licence
