The pay-rate for the role will range from $42.15-$44.24/hr depending on your experience.
About the role:
We are seeking a full-time permanent passionate and skilled Client Liaison to join our Logan service and make a meaningful impact in the lives of our clients. The overall purpose of the Client Liaison is to engage in client relationship management and packaged care case management with clients, their carer/s, family and advocates for home care and disability services.
* Provide the first point of significant contact for the client with the intent of building client trust and confidence in Anglicare to facilitate Consumer Directed Care.
* Undertake home visit to deliver a consumer induction; gather information to identify and understand the client's needs and preferences, establish or meet goals and aspirations, and assess/monitor the client's capacity and interest in the control of their package.
* Implement the client's Agreement to ensure it remains relevant and current and is in line with
* Anglicare requirements for home and/or disability package guidelines.
* Build collaborative relationships with clients, family, carers and significant others.
* Act as the client's primary contact for all matters relating to delivery of services.
* Maintain a client-centered approach in all engagements with clients.
About you:
* Possess clarity in attitudes based on respectful relationships which reflect a commitment to instilling hope and sustaining capabilities of the client through solution focused practice.
* Demonstrated proficiency in Client Relationship Management (the ability to build trust, maintain rapport, form collaborative relationships with clients, carers, significant others)
* Demonstrated case management skills (assessment, support planning, review, advocacy)
* Experience with a community services, or similar organisation and ability to demonstrate a sound understanding of the needs and experiences of people who receive community care services and of the community care system.
* Ability to communicate verbally and in writing to a range of stakeholders in a clear and structured way.
* Ability to apply knowledge of organisation processes and resources to assist clients.
* Experience in or ability to acquire skills in monitoring and/or analysis of expenditure to budget and working within set budget parameters.
* Demonstrated negotiation and conflict resolution skills including the ability to negotiate between individuals, communities and organisations.
* Knowledge and experience in the Microsoft Office suite and exposure and/or ability to acquire skills in a consumer database.
Role: Requirements:
* Current National Police Certificate
* Current Drivers Licence and own reliable vehicle
* Dependent on Service based staffing models, this role is required to possess:
o Minimum certificate 4 qualification - Certificate IV Ageing, Cert IV in Lifestyle & Health, Certificate IV Disability, or Diploma Enrolled Nursing and;
o Attainment of nationally recognised competency in case management or equivalent or willingness to attain
