Blacktown and Mount Druitt Hospitals are part of one of the largest health services in NSW, delivering comprehensive care to a rapidly growing and diverse community. Our Pharmacy Department is committed to excellence in patient care, innovation and continuous improvement.

You will join a collaborative multidisciplinary Heart Failure Service, working alongside cardiologists, nurses and allied health professionals to support complex cardiac patients through specialised outpatient care.

What you'll be doing
As the Heart Failure Clinic Pharmacist, you will provide specialised clinical pharmacy services to patients attending outpatient heart failure clinics, supporting safe and effective use of medicines and improving continuity of care across healthcare settings. Applicants must be available on Tuesdays and Thursdays, which are designated Heart Failure Clinic days.

• Deliver comprehensive clinical pharmacy services including medication review, reconciliation and patient education
• Optimise guideline directed heart failure pharmacotherapy in collaboration with the multidisciplinary team
• Support safe transitions of care between inpatient, outpatient, primary care and community pharmacy settings
• Provide leadership, mentorship and guidance to pharmacists, technicians and students
• Contribute to quality improvement, service development, research and multidisciplinary care planning

Essential Criteria

* Qualification: Pharmacy qualifications conferred by Tertiary Education Institutions e.g Bachelor of Pharmacy. Must satisfy Grade 2 classification as per Health Employees Pharmacists (State) Award.

* Registration: Full and unrestricted Pharmacist Registration with AHPRA





Selection Criteria

* Current registration as a Pharmacist with the Pharmacy Board of Australia via AHPRA. Demonstrated interest in developing advanced clinical skills within a hospital setting, particularly in the care of heart failure patients and other complex chronic disease groups.

* Recent experience working in a hospital pharmacy department, including exposure to inpatient or outpatient clinical services. Demonstrated interest in expanding knowledge of heart failure management, medication optimisation and multidisciplinary models of care

* Well developed organisational and time management skills appropriate to a busy hospital environment, with the ability to prioritise clinical tasks and participate in the delivery and improvement of hospital based outpatient pharmacy services under supervision

* Developing clinical problem solving skills with the ability to recognise medication related issues commonly encountered in hospitalised and ambulatory care patients. Willingness to learn and apply guideline directed therapy principles for heart failure within a structured and supervised hospital framework

* Demonstrated ability to communicate effectively with patients, carers and members of the multidisciplinary hospital team, including medical, nursing and allied health staff. Commitment to contributing to a positive, collaborative hospital culture and supporting peers, students and pharmacy support staff as appropriate

* Demonstrated commitment to ongoing professional development relevant to hospital pharmacy practice, including participation in education, training and formal or informal learning related to cardiology, heart failure, chronic disease management or other acute care areas
