Employment DetailsType: Permanent Full TimeClassification: Nurse PractitionerRemuneration: $139,661.16 - $149,538.56Hours per week: 38Requisition ID: REQ566662Application closing date: 5th May 2025Additional RequirementsEmployment with Mid North Coast LHD requires candidates to hold a valid authority to work in Australia (Aust Work Rights Visa).Role OverviewThe Rural Generalist Aboriginal Health Nurse Practitioner/Transitional Nurse Practitioner (NP/TNP) provides high-level, clinically focused care across emergency, inpatient, chronic, complex, and primary care services at specific sites.Key ResponsibilitiesProvide holistic care for a wide range of acute and chronic conditions across all ages to meet community needs.Facilitate integrated care through referrals, contribute to clinical excellence via research, teaching, consulting, and leadership in nursing.Work autonomously and collaboratively within an extended nursing role, managing patients with chronic and complex conditions.Offer expertise in complex health conditions, develop emergency/inpatient care plans, and coordinate discharge plans for high-risk Aboriginal and Torres Strait Islander patients.Establish culturally safe partnerships addressing community priorities, including diverse population groups.Coordinate healthcare activities, referrals, and liaise with community agencies relevant to patient conditions.The NP/TNP role embodies nursing values, knowledge, and practices, delivering innovative healthcare that complements other providers.
