Overview
Job Summary: The Nurse Practitioner provides advance practice nursing care to patients, families and the community through assessment, planning, prescribing and evaluation interventions. A Nurse Practitioner is a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution. The Nurse Practitioner exemplifies the Upstate mission, vision and values and acts in accordance with Upstate Policies And Procedures.
Minimum Qualifications
NYS licensed/registered NP (NYS NP specialty area of certification must be consistent with practice). Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire (eligibility criteria for NP requires National Certification through AANP or ANCC, and Master\'s Degree). Initial certification must be present or obtained within 6 months of hire. Current BLS certification through the American Heart Association or the American Red Cross is required.
NOTE: Any Nurse Practitioner credentialed through SUNY Upstate Medical Staff Services before January 1st 2013, will not be required to obtain National Certification, although maintenance of current certification is strongly encouraged.
Preferred Qualifications
Prior experience in specialty area of Occupational Medicine/MSK/Ortho.

Monday-Thursday 8:30 am - 5:00 pm
Salary
L5: $92,000 - $147,200
Salary information shown is a general guideline only. Salaries are based upon candidate skills, experience, and qualifications, as well as internal equity, market and business considerations. The salary range depicted is based on full time, 1.0 FTE and would be prorated accordingly for less than 1.0 FTE.
