",{p}We are seeking an experienced and skilled Community Nurse to join our Integrated Neighbourhood Teams in Ipswich and East. As a key member of our team, you will be responsible for providing high-quality patient care and contributing to the development of local health and social care plans.

Key Responsibilities:
* To provide holistic and sometimes complex nursing care to patients within their own homes.
* To work closely with other healthcare professionals, including Community Nurses, District Nurses, Generic Support Workers, Physiotherapists, Occupational Therapists, Social Workers, Independence Wellbeing Practitioners, Social Prescribers, Matron, Care Coordinators, and Team Administration Colleagues.
* To contribute to the development of local health and social care plans through data analysis and delivery of local delivery plans.
* To participate in the co-location of services with our social care colleagues and wider Multi-Disciplinary Team (MDT), providing benefits to patient outcomes.
* To work within a diverse caseload, led by a Band 7 Clinical Lead and three full-time Band 6 Senior Nurses.
* To be flexible, motivated, and able to work within a changing environment.
Required Skills and Qualifications:

* Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
