As an Outreach Support Worker, you'll be supporting incredible individuals living with neurological conditions like MS, Parkinson's, Huntington's, MND, ABI, and Stroke promoting and fostering community engagement.
You'll be involved in assisting several engaging activities (e.g., arts & crafts, board games, gardening, quizzes, and team games), in a safe and social environment that allows our clients to interact and build social connections.
What You'll Be Doing
* Lead and support outreach activities that promote social connection and engagement
* Assist the Kitchen Supervisor in preparing and serving meals, ensuring a welcoming dining experience
* Deliver exceptional service with a strong focus on enhancing the client experience
* Follow individual care plans to meet each client's unique needs and preferences
* Provide mobility assistance to support safe and comfortable movement
* Stay attuned to emotional and physical needs, ensuring care is always person-centred and responsive
About you
You're a compassionate, reliable, and proactive individual who's passionate about making a difference in the lives of others. You thrive in a team environment and are committed to delivering high-quality, person-centered care.
Essential
What You Bring:
* A genuine passion for supporting people with disability or the frail aged
* Strong communication skills—both written and verbal
* Confidence using systems and technology in your daily work
* NDIS Worker Screening Check and NDIA Orientation module (or willingness to obtain)
* Current provide First Aid and CPR Certificate
Desirable
* Certificate III in Individual Support, Disability, Aged Care, or a related field
* Experience working in centre-based settings or with people with disability
