As a TCP Coordinator, you’ll play a key role in coordinating client care, working with a multidisciplinary team, and ensuring seamless transitions across hospital, residential, and community settings. As the Transition Care Program (TCP) Coordinator, you will play a vital role in supporting older individuals as they transition from hospital to community-based care. Working closely with a skilled and supportive team of clinicians you will coordinate the acceptance of clients into the program and ensure they receive tailored support that meets their unique needs. Your responsibilities will include liaising with hospitals to identify suitable patients, developing short-term care coordination plans, and facilitating smooth transitions to rehabilitation services. In this dynamic position, you will take accountability for your practice standards while mentoring and guiding less experienced staff. You will also be instrumental in establishing ongoing case management in the community and maintaining essential linkages with aged care agencies, primary health care, and General Practice. Your leadership will help ensure that our older patients receive the comprehensive care they need for a successful recovery and reintegration into their community. About You We're looking for a dedicated professional with a strong background in Allied Health. If you meet the following criteria, we want to hear from you! Qualifications: Degree in a relevant Allied Health discipline with registration eligibility. Communication Skills: Effective in problem solving, conflict resolution, and team collaboration. Experience: At least 3 years post-graduate experience in Allied Health, particularly with older adults. Holistic Approach: Ability to develop and implement community-focused therapeutic interventions.
