**About the Opportunity**
South Western Sydney Local Health District is seeking a dedicated, outgoing and purpose driven Senior Social Worker (Level 3) to join the district-wide multidisciplinary Commonwealth Home Support Program (CHSP) team.

This is a new role with responsibility for clinical governance of Level 1/2 Social Workers within Ageing Well in Community Allied Health services, as well as carry their own clinical load.

As the Senior Social Worker you will have a keen interest in professional development, clinical supervision, teaching and quality improvement activities including service promotion and development activities.

This role is an amazing opportunity for a Senior Social Worker to build on their experience by working with a diverse client base of older people living in the community. Through the CHSP program you will be working with clients who have a wide range of challenges, and you will be able to work with clients with specific goals in mind. You will be working with clients on reabling them in their homes to achieve their goals. Whilst working with the clients you will have the opportunity to view their life as a whole and work with them to achieve their vision of reablement. You will have the chance to work with families and the wider community to provide a holistic approach to the client care.

**What You'll be Doing**
This position is responsible for providing specialist Social Work service to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

**How to Apply**
- A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).Extensive post graduate experience working as a Social Worker in relevant clinical areas, including a demonstrated understanding of the health setting, complex health conditions and the impact on wellbeing.
- Demonstrated experience in the provision of clinical supervision and expert advice/consultation to the social work service and rehabilitation, aged care and/or community settings.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
