**About the Role**

We are seeking a motivated experienced Community Development worker with excellent communication skills to join the Complex Care Addiction Medicine Unit who operate within the Community Assessment and Response Team (CART). The Community Development Worker ensures that the patients/clients presenting to hospital with alcohol or drug related issues assists and provides appropriate care that transitions them back into the community. Essential to the role is addiction medicine experience as well as an understanding of the community services available to them. We operate from a person - centred care framework and strongly align with the social model of health.

**About You**

You are a dynamic Community Development Worker, with a genuine passion and interest in this unique opportunity and will commit to:

- Work across various Monash Health sites as required.
- Have sound skills in assessing and planning patient's community health discharge requirements.
- Current knowledge of various community health support services.
- Current experience in providing an outreach service to this cohort

**Looking after Our People**:

- Ensuring you work within the scope of practice as a Registered Nurse, and escalating to the appropriate management line, Nurse Practitioners and Medical Professionals when required.
- Completing all mandatory and competency assessment training.
- On a personal level, you are enthusiastic and motivated and demonstrate excellent communication and interpersonal skills. You are willing to commit to team work and ongoing professional development that align with the Monash Health's values and vision. **What You Need**

Essential:

- Demonstrated recent experience as a Registered Nurse with at least 5 years post graduate experience.
- Current AHPRA registration
- Australian working rights
- Valid Driver's licence
- Current Working with Children's Check
- Current Police Check
- Desirable_:

- Previous Community experience
- Post-graduate qualifications in Addiction Medicine
