Join us at Mercy Hospital, and help us deliver even greater outcomes for our patients, families, communities, and you. **About the opportunity** Mercy Care Centre Young is a 26-bed subacute health service facility providing a range of outpatient, community health and aged care services for the people of Young and surrounding region. It specialises in geriatric evaluation and management; rehabilitation and palliative care. Our facility is co-located with the Young Health Service and sits within Murrumbidgee Local Health District.* Provide quality patient- focused and clinical care utilising a person-centred approach* Undertake assessments, treatment and documentation to the highest standard* Work on our 26 bed subacute ward specialising in rehabilitation & palliative care* Be part of a multi-disciplinary team which requires you to provide a highly responsive and flexible service to consumers and carers.**What you'll bring** You're resilient, compassionate and have a whole lot of heart. To thrive in this role, you will have:* Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.* A compassionate, flexible and positive approach to effective patient care and management.* The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.You will also be required to provide evidence of, or in the process of obtaining:* Current registration with APHRA* Recent experience in palliative care and or rehabilitation desirable* Demonstrated clinical ability and skills* Current Immunisation of diseases as listed in NSW Health Policy Directive.
