HealthWISE is seeking compassionate and motivated Mental Health Support Coordinators to join our team delivering suicide prevention and aftercare support across the New England North West region. Use your lived experience/peer worker qualification to deliver suicide prevention and aftercare support (this is NOT an NDIS role).
This role focuses on providing rapid and supportive follow-up for people who have attempted suicide, helping them stay safe, connected and supported during their recovery journey.
Working alongside clinicians and community providers, you will provide non-clinical psychosocial support, helping people strengthen social connections, build resilience, and access services that support their wellbeing.
Support Coordinators work closely with individuals, families and carers to develop Safety Plans and Support Plans, facilitate engagement with services, and bridge gaps between hospital, community and outpatient care.
This role is ideal for people with a Certificate IV or Diploma in Mental Health, peer worker qualification, or relevant lived experience, who are passionate about recovery-focused and trauma-informed support.
The position offers flexibility in service delivery, including telehealth and community-based support, and the opportunity to work as part of a collaborative team environment.
