Job Opportunity: Disability Navigator

* As a key member of our multidisciplinary team, you will play a pivotal role in improving the care experiences for people with disability, their families and informal supports.
* Utilizing your expertise in health and disability sectors, including knowledge of National Disability Insurance Scheme (NDIS), you will coordinate care and navigate systems for individuals with complex needs across multiple services and agencies.
* You will collaborate with healthcare teams to address disability-related needs, establishing reasonable adjustments and supporting discharge planning.
* Moreover, you will work closely with external partners to ensure the care needs of people with disabilities are met, thereby reducing acute health responses.
* Providing guidance, leadership and advice to facilities, you will support the needs of people with disabilities who present to hospital.
* Implementing proactive care navigation and hospital avoidance strategies, you will build and strengthen networks with healthcare services that support the needs of people with disability.
Key Responsibilities:

* Work collaboratively with healthcare teams and external partners to ensure the care needs of people with disabilities are met.
* Provide guidance, leadership and advice to facilities to support the needs of people with disabilities who present to hospital.
* Implement proactive care navigation and hospital avoidance strategies for people with disabilities with unmet/unstable care needs.
* Build and strengthen networks with healthcare services that support the needs of people with disability.
Requirements:

* Relevant qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist or Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA) OR qualified Social Worker eligible for membership with the Australia Association of Social Workers (AASW) OR qualified Speech Pathologist eligible for membership with Speech Pathology Australia OR CNC Grade 1: At least 5 years full time equivalent post registration experience and in addition who has approved post registration nursing/midwifery qualifications relevant to the field in which the employee is appointed, or such other qualifications or experience deemed appropriate by the public hospital or public health organisation.
* Extensive knowledge of the health and disability sectors, including specific experience with the National Disability Insurance Scheme (NDIS).
* Ability to coordinate care and navigate systems for individuals with complex needs across multiple services and agencies.
* Strong communication and interpersonal skills.
* Australian C-Class driver's license (including P1 or P2).
