* URGENT Wide Bay, QLD - Locum Hospital Pharmacist - locum rates upto $80 per hour + super
* HP3 or HP4 Hospital Pharmacist required for urgent locum in Wide Bay, Queensland. You will be covering a clinical workload consisting of rehabilitation (physical rehab - not drug and alcohol) e.g post stroke, post trauma, reconditioning etc) and geriatric medicine.
* URGENT NT - Locum Hospital Pharmacist - 18th September for 6-8 weeks. Ward based Pharmacy, join a small team of three. Knowledge of renal and indigenous health highly regarded.
* Coastal VIC 2 x Locum Hospital Pharmacists - ASAP start for 3 months ++ with locum rates and travel assistance of $200
* 2 x Locum Hospital Pharmacists required for a coastal service. Full-time, 80 hours per fortnight. Grade 2 level (open to experienced Grade 1 clinicians) with caseload, supervision and junior clinician support.
* Mackay, QLD - Locum Hospital Pharmacist - locum rates upto $75 per hour + super
Role Details
* You will be responsible for the direct provision of pharmaceutical services to patients and staff of the service, including inpatient and outpatient dispensing, supervision of technical staff, ward-based clinical services, provision of medicines information and education services to clinical staff.
* The Pharmacy team oversees formulary and medication supply for the hospital including controlled and highly specialised substances.
* Work on rotation across acute ward, subacute ward, outpatient specialty centres (dialysis and chemotherapy) and the urgent care centre.
* Provide a pharmacy service including resolving medication-related issues, supplying medications, completing the Medication Reconciliation Process, providing medicines information, counselling patients and assisting the discharge process.
* Oncology pharmacy service as available: review treatment plans, ordering chemotherapy, preparation of supportive medications and counselling chemotherapy patients.
* Ensure appropriate drug utilisation through hospital policy and formulary guidelines, liaising with medical and nursing staff; assist team members in delivering a pharmacy service.
* Monitor and assess medicine management to enhance medication safety in accordance with Standards established by Advanced Pharmacy Australia.
* Manage education needs of patients and staff to improve knowledge, adherence and safety.
* Provide medicine-related education to staff to enhance knowledge, medication safety and quality use of medicines.
Qualifications
* AHPRA registration and Australian hospital pharmacy experience required
* Australian hospital experience is mandatory
* Experience in a hospital setting and ability to work in a team
