About the Role

As an APS6 Clinical Care Coordinator, reporting to the Assistant Director, you will deliver short‑term, goal‑oriented clinical case management to clients with complex health and psychosocial needs that impact their ability to access or meaningfully engage in mental health care.

Working within a stepped care model, the role focuses on promoting engagement with mental health services by identifying appropriate supports, reducing barriers to access, and addressing psychosocial and health factors that may lead to deterioration in mental health if left unaddressed. Services are delivered one‑to‑one, with a strong emphasis on worker–client alliance and involvement of family and carers where appropriate.


Key responsibilities include:

* Provide high‑quality, evidence‑informed clinical services including intake, assessment, brief intervention, risk management, care coordination and referral for a caseload of clients with complex and/or multiple needs.
* Maintain accurate and confidential client records using the Client Management System, including care plans, risk assessments, progress notes and documentation of client interactions.
* Deliver trauma‑informed, recovery‑oriented and client‑centred care, incorporating focused psychological strategies such as psychoeducation, problem‑solving, distress tolerance and motivational interviewing.
* Develop and maintain effective internal and external relationships with stakeholders including government agencies, community mental health and welfare services, ex‑service organisations and other relevant service providers.
* Work systemically with internal and external stakeholders to support a recovery‑focused approach to service delivery within a stepped care model for vulnerable or at‑risk communities.
* Participate in regular case reviews, clinical supervision and Continuing Professional Development (CPD) to maintain professional registration.
* Adhere to relevant legislation, ethical standards, policies and procedures, including risk escalation pathways and documentation requirements.
* Respond to critical incidents in a calm, timely and clinically appropriate manner.
* Contribute to additional clinical and service delivery activities as required, including group programs, intake assessments, service allocation processes, community development, program evaluation and quality improvement initiatives.


About You

You are a skilled and compassionate clinician with experience supporting individuals with complex needs and a strong commitment to evidence-based, trauma-informed practice. You will demonstrate:

* Excellent interpersonal skills and the ability to deliver holistic, client-centred intake, assessment and brief intervention with vulnerable clients.
* Strong clinical assessment and risk formulation skills, with the ability to implement appropriate and timely risk management strategies.
* High-level written and verbal communication skills.
* Sound judgement and the ability to respond effectively to critical or high-risk situations.
* Strong IT capability and confidence using clinical systems and digital platforms.
* Commitment to ethical practice and adherence to relevant policies and procedures.
* Australian citizenship and the ability to successfully complete pre-engagement screening and obtain and maintain a Baseline security clearance.
* Ability to obtain and maintain a Working with Vulnerable People registration or a Working with Children Check.
* One of the following professional qualifications:
o Registered Psychologist, Occupational Therapist or Registered Nurse with full AHPRA registration, or
o Social Worker with membership of the Australian Association of Social Workers (AASW), or
o Masters-level counselling qualification with registration as a Registered Clinical Counsellor (PACFA) or ACA Level 3 or Level 4.
