The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).
Join our interdisciplinary Community Supportive Care team supporting clients in the last two years of life who are living with late‑stage degenerative or chronic conditions. The Senior Occupational Therapist focuses on promoting independence at home, coordinating care with community teams, GPs, hospital services and specialist palliative care, and providing vital support to families and carers.
The successful incumbent will work autonomously, develop and review care plans, manage own caseload and ensure timely access to services such as SASH, My Aged Care and Enable. In this position, you will use advanced clinical reasoning to provide recommendations on daily living, assistive technology, environmental modifications, pressure care and carer education, while maintaining strong communication with clients, carers and service providers.

What You Will Bring To The Role
* Degree in Occupational Therapy and current registration with Occupational Therapy Board of Australia (AHPRA).
* Demonstrated high level of clinical expertise and knowledge in the assessment and management of clients with chronic and complex conditions including experience in palliative care across community or hospital settings.
* Demonstrated leadership ability to provide clinical support, education, and consultation to health professionals in area of specialty to improve client outcomes and service design.
* Demonstrated problem solving and decision-making abilities according to best practice including the ability to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
* Demonstrated ability to communicate effectively with clients, carers and stakeholders and provide advice to managers on service development.
* Demonstrated commitment to evidence-based practice and quality improvement.
* Ability to work independently with minimal supervision and maintaining involvement within the wider interdisciplinary team.
* Current driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD.
