About the Role

The Social Work Department is looking for a Grade 2 Social Worker to work on the General Medicine Unit at Monash Medical Centre, Clayton. This role is full-time, permanent and you would be joining a supportive team of eight social workers in the General Medicine stream.

As a Grade 2 Social Worker at Monash Health, you will play a vital role in delivering high-quality, evidence-based social work services to patients and their families. This role involves managing a clinical caseload across designated areas while maintaining and building clinical expertise. You will apply advanced psychosocial assessment and planning skills, drawing on best practice and evidence to guide your interventions.

You'll work collaboratively within a multidisciplinary environment and be expected to navigate and integrate service systems such as Family Violence, Child Protection, My Aged Care, the Transition Care Program, NDIS, and Mental Health services. The role also includes active participation in quality improvement initiatives and research projects. Your work will align with AASW professional standards, including the Code of Ethics, Supervision Guidelines, and Scope of Practice.

About Us

The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists. The Social Work Service operating hours are standard business hours Monday to Friday. A weekend service is provided by roster at Casey, Clayton, Dandenong and Victorian Heart Hospital sites and an on-call service is also provided to the acute sites.

About You

You are a passionate and skilled Social Worker who is ready to take the next step in your career. As a Grade 2 clinician, you bring solid experience in delivering specialised clinical services and conducting complex psychosocial assessments. You thrive in dynamic healthcare settings and are committed to improving outcomes for patients through advocacy, collaboration, and innovation.

You're keen to grow your leadership capabilities, take on clinical responsibilities, and contribute to service development through project or research work. With a sound understanding of key service systems and a recovery-oriented approach, you're ready to make a meaningful impact at Monash Health.

Qualification & Licence requirements include:

* Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
* Eligible for ordinary membership of AASW if overseas trained )
* A current Victorian Driver's Licence
