About The Role
Our goal is to provide a service that builds 'healthier rural people and thriving communities'. In this role, you will develop long-term relationships with patients, simplifying referral pathways and access to care to help them manage their health within their communities.
This rewarding role involves engaging with people's lives beyond their illnesses. As part of the multidisciplinary care team, you will deliver nursing care in community and ambulatory settings, and lead, direct, and supervise other care roles, including nurses, students, and Aboriginal Health Workers, to ensure quality care.
The successful candidate will have knowledge and experience in chronic disease prevention, health improvement initiatives, and evidence-based hospitalisation prevention strategies to enhance the health and wellbeing of the Western NSW community.
