ROLE
We are seeking an experienced mental health Clinical Care Coordinator to make a meaningful impact through client support and care coordination.
In this role, you will manage complex cases, navigate service systems, and liaise with a range of stakeholders including government, community, and organisational partners to deliver seamless client support.
* Full-time role, 37.5 hours per week
* Contract Length: Initial 24-month contract with the possibility of extension
* Location: Winnellie & Palmerston NT

SOME KEY RESPONSIBILITIES
* Manage complex client cases and coordinate high-quality support
* Help clients and families navigate services and access resources
* Liaise with government, community, and partner organisations to ensure seamless care
* Maintain accurate client records, care plans, and monitor progress
* Contribute to program improvement, quality assurance, and stakeholder feedback initiatives
REQUIREMENTS
* Australian Citizenship
* Ability to obtain a Baseline Security Clearance
* Current (or ability to obtain) Working with Children / Vulnerable People check
* Professional registration/membership in one of the following:
* AHPRA registered Psychologist, Occupational Therapist, or Nurse
* Social Worker with AASW membership
* Masters-qualified Counsellor with PACFA or ACA (Level 3/4)
