**About the Role**

The Aboriginal Access and Support Worker delivers a home and community care program for Aboriginal people aged under 50. The program provides basic support and maintenance services for Aboriginal people who experience difficulty performing the activities of daily living and help people to remain independent at home and in the community. The program aims to encompass cultural, emotional and spiritual support to Aboriginal clients and their families. The Aboriginal Access and Support Worker works closely with internal and external health professionals to actively contribute to the goals and health plans. Key result areas:

- Promote the program within the Aboriginal community by engaging with the local members including organisations that work with Aboriginal people within the Dandenong and Casey Local Government Area.
- Provide information about the range of health care services available to the Aboriginal and Torres Strait Islander people and link their needs to identified services.
- Provide assertive outreach to identify Aboriginal and Torres Strait Islander people eligible for Monash Health services.
- Work collaboratively with funded services, Aboriginal Controlled organisations and other key organisations to facilitate improved client access and support.
- Provide secondary consultation to non-Aboriginal staff working with Aboriginal and Torres Strait Islander patients/clients and families.
- Promote safe and appropriate cultural practices to meet the needs of the Aboriginal and Torres Strait Islander communities.
- Support Aboriginal and Torres Strait Islander people and their families in their health care journey in the hospital and community and across the life span.
- Support Aboriginal and Torres Strait Islander people through the Outpatient process and appointments.
- Provide support to Aboriginal and Torres Strait Islander people to access the My Aged Care system and NDIS scheme, ensuring individuals and families are supported to access the care they require, providing advocacy when necessary.

**Responsibilities**:

- Actively participate and work collaboratively with other members of the Aboriginal Health Service.
- Advocate for the rights of Aboriginal clients and their families.
- Assist Aboriginal clients to access and navigate the health care system.
- Provide advice, information support and basic counselling as required for clients and their families.
- When relevant, ensure individuals are supported through the My Aged Care's registration, screening and assessment process, undertaking the role of the regular representative when requested.
- Actively provide services available to the community on how to remain in their home independently as they age.
- Promote the Aboriginal Health Service across the organisation.
- Maintain complete and accurate notes in HMS, SMR and EMR and other patient data management systems.
- Produce reports, provide statistics and participant on committees as required.
- To contribute to health promotion activities.
- Maintain daily worksheets, car logbook, diaries and statistics.
- Make reports of progress to the manager and report formally through the Point of Care report.
