**About the Opportunity**
SWSLHD Transitional Aged Care Program (TACP) is seeking a Level 1 / 2 dietician to support their community and/or residential teams in providing high quality clinical dietetic assessment and intervention to older adults who are accessing the program.
TACP is a unique program focused on the reablement of older adults living in the community. Working with clients, their carer’s, the TACP multidisciplinary team and other relevant stakeholders, you will be contributing to holistic care that ensures better health outcomes and quality of life over a time-limited period post-discharge from hospital.
Office is located in Braeside Hospital.
**What You'll be Doing**
This position is responsible for providing patient centered and high quality nutrition services to patients in SWSLHD including providing nutritional care to patients as per caseload; provide and participate in education and evaluation; supervise students as appropriate; and participate in ongoing professional development, quality improvement and research as required.

**How to Apply**
- Post graduate qualification or Bachelor in Nutrition and Dietetics as recognized by the Dietitians Association of Australia.
- Relevant clinical experience and good knowledge of current Dietetics Practices.
- Excellent written and verbal communication skills, including group education skills.
- Demonstrated ability and commitment to quality improvement
- Demonstrated ability to work independently and as part of the team
- Demonstrated commitment to continued professional development and evidence based practice
- Excellent computer skills including CBORD
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
