We're seeking an experienced and compassionate Clinical Care Partner to provide high-quality clinical leadership and care coordination within our Community Aged Care program.
About the Role
As a Clinical Care Partner, you will work closely with participants, their families and key health professionals to deliver safe, person-centred, in-home care. You will be responsible for comprehensive clinical assessments, care planning, ongoing review, and clinical oversight, while supporting staff and managing emerging clinical risks.
This is a varied and rewarding role that combines direct clinical expertise, leadership, case management and quality improvement, supporting some of our most complex participants.
Key Responsibilities
* Conduct comprehensive nursing assessments and develop, implement and review individualised care plans
* Provide clinical oversight to ensure safe, high-quality service delivery
* Monitor and respond to changing clinical risks, initiating reassessments as required
* Coordinate care with multidisciplinary teams and external providers
* Educate participants and families on care plans, rights and service agreements
* Provide leadership, supervision, coaching and training to support workers and team members
* Support complex case reviews, case conferences and incident responses
* Maintain accurate documentation and ensure compliance with funding, clinical and quality standards
* Contribute to service development, audits, continuous improvement and staff education
* Promote wellness, reablement and person-centred care approaches
* Assist with intake, new referrals and allocation of workloads within agreed KPIs
About You
You are a confident and compassionate registered nurse who thrives in community-based care and enjoys balancing clinical excellence with leadership and collaboration.
You will bring:
* Bachelor of Nursing and current, unrestricted AHPRA registration
* Experience in community or aged care case management
* Strong knowledge of the aged care system and Strengthened Aged Care Standards
* Experience supervising, mentoring or training staff
* Excellent assessment, communication and problem-solving skills
* Ability to manage competing priorities and complex caseloads
* Strong documentation, organisational and computer skills (MS Office, client management systems)
* A person-centred, ethical and culturally respectful approach to care
Requirements
* Current First Aid & CPR
* National Police Check and NDIS Worker Screening Check
* Full unrestricted driver's licence and reliable vehicle
* Willingness to work flexibly, including on-call and out-of-hours support as required
