As a Support Coordinator, you will collaborate with NDIS participants and their networks to identify goals, access services, and promote self-sufficiency.
* Implement supports from NDIS plans including formal, mainstream and funded services.
* Assist participants to understand their plan funding, negotiate services and monitor delivery.
* Manage time-limited specialist coordination for high-risk situations, and provide crisis support when needed.

This role requires strong interpersonal skills, the ability to work autonomously, and a passion for delivering exceptional support.
You will be responsible for:
* Building rapport with NDIS participants and their families.
* Maintaining accurate records of participant progress and outcomes.
* Collaborating with internal stakeholders to ensure seamless service delivery.
