Care Navigator
Medicare Mental Health Centre provides high-quality, evidence-based assessment, care coordination, information, support, and treatment to individuals experiencing psychological distress, their family members, carers, and support networks.
The Care Navigator role facilitates person-centred and integrated care by working collaboratively with consumers to connect them with services and supports in the community. This position delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time.
* A tertiary qualification in a relevant discipline is desirable, however extensive experience in mental health, health, community services or social sciences is also acceptable.
* Understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems is essential.
* Knowledge of supports and services available in local and regional communities is required.
* Ability to engage with consumers and their families or carers to provide care navigation and referral support is crucial.
This is a full-time opportunity with the option for a Rostered Day Off each month. The service will operate seven days a week, and candidates should be flexible and available to work on a rostered basis, including on weekends and outside of normal business hours, as needed.
