About the Role
As a Social Worker with the Metropolitan Palliative and Community Care Service (MPaCCS), you will:
* Support and mentor staff delivering palliative and end-of-life care
* Facilitate advance care planning, case conferencing, and shared decision-making
* Provide guidance on effective communication at the end of life
* Deliver culturally safe care, including inclusive practice for LGBTI communities
* Connect staff, patients, and families with relevant support services and resources
About You
We are seeking an experienced and empathetic Social Worker who is passionate about person-centred care and making a difference in the community. You will bring:
* At least 2 years' experience in a health services or related setting
* Advanced skills in social work, mentoring, and relationship-building
* Knowledge of advance care planning, case conferencing, and shared decision-making
* Strong communication and problem-solving skills, with the ability to work both independently and as part of a multidisciplinary team
* An understanding of available end-of-life resources and services
What You Need
* Bachelor or Master of Social Work (essential)
* Current WA Driver's Licence (C-class, unrestricted)
* National Police Clearance (or willingness to obtain)
* Full Australian working rights

About Bethesda Health Care and MPaCCS
Bethesda Health Care is a private, not-for-profit hospital known for delivering outstanding care, including one of Perth's leading inpatient palliative care units. MPaCCS, based in South Perth, provides specialist consultancy and works closely with GPs, residential aged care facilities, and other providers to deliver high-quality outcomes for patients and their families.
