Care Coordination Specialist
We are seeking a dedicated and compassionate Care Coordination Specialist to support children and young people in out-of-home care. Our Permanency Support Program offers a range of services designed to meet the diverse needs of these individuals.
Main Responsibilities:
* Effectively manage a case load and meet individual targets.
* Develop, implement and review individual plans to address the emotional, social, behavioural, developmental and educational needs of children and young people.
* Safeguard their wellbeing and report any concerns immediately.
Requirements:
* Tertiary qualification in social work or a relevant field is required.
* Demonstrated experience in casework practice, preferably in Out-of-Home-Care, is highly valued.
* A proven ability to work with children and young people who have social, emotional and behavioural difficulties is essential.
