Job Overview
The purpose of the Child and Family Practitioner role is to provide skilled support and supervision to foster carers in a family-based care arrangement. As a key member of our team, you will work in partnership with government agencies, children, young people, and their families to ensure the highest standards of care are met.
This is a full-time permanent position based in Mackay.
Key Responsibilities:
* Undertake casework practice (case planning and assessment) in partnership with relevant government agencies, children, young people, families carers other stakeholders ensuring compliance with QLD government requirements.
* Provide ongoing support supervision to Foster Carers regarding caring for children young people at home including facilitating case planning processes implementing strategies outlined within individualised care plans supporting direct reports.
* Evaluate assess necessary risk assessments cultural support plans safety plans behavioural support plans ensuring all documentation information such as carer renewals foster care agreements file notes records are up-to-date including inputting data into relevant databases.
1.
