In this role, you will deliver person-centred support that helps people with disabilities lead more independent and meaningful lives within the Portland community and nearby areas.

Key Responsibilities
In this role, you will:
* Assist clients with personal care and everyday activities
* Support community participation and social interaction
* Help with meal preparation, medication reminders, and light household tasks
* Follow and implement individualised support plans
* Maintain a safe and positive environment
* Build strong, supportive relationships with clients
* Work collaboratively with the wider team
Requirements
To be considered, you will need:
* Certificate III in Individual Support (or equivalent)
* NDIS Worker Screening Check
* Working with Children Check (or willingness to obtain)
* Current First Aid and CPR
* Police Check (within the last 6 months)
* Valid Australian driver’s licence
* Reliable vehicle with appropriate insurance
Preferred: At least 1 year of experience in disability support
