Your Opportunity
Reporting to your Dietetics Team Leader, you will provide high-quality mobile dietetics services to adults living in Residential Aged Care facilities and the local community.
Responsibilities
* Providing person-centred dietetics care to all clients
* Helping to improve nutrition and hydration systems in residential aged care homes
* Delivery of educational sessions
* Completing menu and mealtime assessments
* Working closely with external stakeholders and Plena colleagues to ensure the delivery of high quality dietetic care
* A commitment to continuous improvement and contemporary best practice
* Completing clinical documentation accurately and in a timely manner
* Contributing to team culture, including actioning the Plena values and actively engaging with dietetics colleagues
Qualifications
* Bachelor's or Master's Degree in Dietetics
* Current DA registration
* Willingness to travel (mobile role)
* Willingness to obtain NDIS workers screening and National Police Check
* Australian working rights
* Proof of Covid-19 and current Flu Vaccination
