 
        
        OverviewPlan & Connect is a trading entity of Regional Disability Advocacy Service (RDAS), which was established to provide independent, local, and trusted support coordination and plan management services to people with disabilities under their NDIS plan. 
With offices in Griffith, Wagga Wagga, Wodonga & Wangaratta, we support people living in Northeast Victoria and Southern New South Wales. 
We are a registered NDIS provider of Plan Management and Support coordination services.Plan & Connect assist people with support coordination and plan management only and are not a NDIS service delivery provider. 
This means we work closely with service providers that match the unique expectations and requirements of the participants that we support.About the RoleOur Support Coordinators work closely with participants and their families to understand their NDIS plan; identify and choose service providers that help them to achieve their goals; connect with and participate in the community and develop skills and capacity in self-management.We have a Support Coordinator role based in our Wangaratta or Wodonga office being a parental leave cover part-time role up to 30 hours per week (Approx November 2025 - June 2026). 
Key ResponsibilitiesWork with participants, families and carers to identify and access opportunities for skills development, capacity in self-management and community participation.Support and encourage participants to achieve their goals and provide support coordination services in line with participant's individual NDIS plans exercising choice and control.Develop networks and collaborate with stakeholders to proactively identify, plan, implement and evaluate supports for participants.Prepare and maintain up-to-date and accurate case management notes and reports in a management database (Careview). 
Ensure compliance with all NDIS practice standards as per our registered provider status.Attend and participate in networking events to promote Plan & Connect services and generate referrals.Achieve billable hours targets.Attend and contribute to team meetings, discussions and planning.Timely completion of reporting for NDIA with supporting documentation from allied health and other supports.The successful candidate will be joining a purpose driven organisation that offers a outstanding base package, salary packaging, flexible working arrangements and YOU days.
