About the Role: As a Direct Care Worker - Community, you provide hands-on support to elderly, frail, or disabled clients wishing to live independently at home. Reporting to the Community Care Coordinator, you work closely with Community Health professionals in a multidisciplinary team to assist with daily living activities and personal care. You are responsible for delivering safe, respectful, and client-centered care in line with care plans, supporting clients’ wellbeing and independence. What We’re Looking For: Completed AQF Certificate III relevant to the position and at least 560 hours satisfactory in-service experience. Demonstrated experience in the provision of a direct care service in a health-related field. Demonstrated experience in dealing with aged, frail or disabled persons.
