Make a Real Difference – Deliver Care at Home:
* Deliver person-centred nursing care in clients' homes.
* Conduct nursing assessments, develop nursing care plans, and complete clinical documentation.
* Mentor and support Home Care Workers in the community.
* Provide clinical advice to clients and families, ensuring dignity, choice, and independence.
* Collaborate with Home Care Managers, Coordinators, and external health professionals.
* Ensure compliance with Aged Care Quality Standards, legislation, and Lutheran Services policies.
About You:
* Current AHPRA registration (Nursing).
* Experience in home care & aged care or community nursing.
* Strong clinical assessment and care planning skills.
* The ability to mentor and support care staff.
* Current Driver's Licence and reliable vehicle.
* Current NDIS Worker Screening or National Police Check.
