Job Title: Respite Care Coordinator


We are seeking a compassionate and experienced Registered Nurse to join our team in providing specialized care to older Victorians. The successful candidate will be responsible for delivering high-quality nursing care, ensuring the physical and emotional well-being of residents, and working collaboratively with our multidisciplinary team.


To succeed in this role, you will possess strong clinical skills, excellent communication and interpersonal abilities, and a commitment to delivering person-centered care. You will also have experience working within an aged care environment and be registered with AHPRA as a Division 1 nurse.


Key Responsibilities:


* Delivering high-quality nursing care to residents, including medication management and wound care;
* Conducting resident assessments, developing individualized care plans, and collaborating with the multidisciplinary team to ensure seamless care delivery;
* Maintaining accurate and comprehensive documentation, adhering to regulatory requirements and organizational policies;
* Providing guidance and support to Personal Care workers and Enrolled Nurses, promoting a culture of teamwork and collaboration;
* Participating in continuous quality improvement initiatives, identifying areas for enhancement and contributing to evidence-based practice.


Requirements:


* Registered Nurse (RN) registration with AHPRA as a Division 1 nurse;
* Experience working within an aged care environment, preferably in a leadership or coordination role;
* Strong clinical skills, including medication management and wound care;
* Excellent communication and interpersonal abilities, with a focus on person-centered care;
* Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently.
