Your opportunity
We are seeking a dedicated Pre & Post Release Case Manager to join our team in Katherine, supporting individuals as they transition from correctional settings back into the community.
In this role, you will provide intensive, person-centred case management, helping participants achieve personal goals, secure stable housing, strengthen social and family connections, and access the services they need to thrive.
This position offers the opportunity to make a tangible difference in people's lives by guiding them through meaningful, evidence-based interventions while contributing to a collaborative, supportive, and culturally aware service environment.
Your key responsibilities will be to:
* Develop and implement holistic, strength-based individualised case plans for participants, including referrals to supplementary services.
* Assist participants in transitioning from correctional settings into the community, including securing accommodation, building life skills, and strengthening social connections.
* Apply specialised knowledge to manage complex participants and support positive behavioural and personal outcomes.
* Respond to referrals, undertake initial registrations, formal assessments, and evaluate participants' suitability for support.
* Contribute to the planning, coordination, and delivery of training programs and activities aligned with participant needs and program guidelines.
* Build and maintain effective relationships with internal teams, external service providers, and community stakeholders to improve participant outcomes.
* Maintain accurate participant records, complete reports and data entry, and ensure compliance with Mission Australia protocols and statutory requirements.
* Undertake occasional travel by road, including to remote locations, to meet participant and program needs.
Requirements for success
* Relevant degree with at least 1 year of experience, or Associate Diploma with relevant experience, or equivalent expertise gained through substantial industry experience.
* Ability to obtain and maintain security clearance to access Northern Territory Correctional Centres.
* Experience working with diverse participant groups, particularly Aboriginal and Torres Strait Islander communities, and understanding cultural protocols.
* Experience in justice or community services sectors, including trauma-informed, person-centred approaches.
* Ability to manage a complex caseload and provide specialised interventions.
* Strong knowledge of statutory requirements and ability to maintain program compliance.
* Excellent engagement, listening, and relationship-building skills with participants and stakeholders.
* Well-developed written and verbal communication, including administrative and reporting skills.
* Positive, person-centred approach with belief in individuals' capacity to grow and self-right.
* Professionalism, including maintaining boundaries and ethical practice.
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
