Role description

Dhelkaya Health is recruiting for a compassionate and capable Care Partner to join our newly developed Support at Home team. This client-focused role will support the delivery of aged care reform as outlined by the Department of Health and Aged Care and provide coordination of in-home care services under the upcoming Support at Home program.

As a Care Partner, you will work directly with older people to assess needs, co-design personalised support plans, and monitor care delivery. This role requires knowledge of the aged care services system, wellness and reablement approaches, and a strong commitment to quality and accountability as per the Aged Care Quality Standards.

Key responsibilities:

- Conduct holistic assessments and develop care plans in collaboration with clients and carers
- Coordinate and monitor the delivery of services, including personal care, domestic support, and social connection
- Advocate for client rights, dignity, and culturally safe care
- Maintain accurate documentation in accordance with legislative and funding requirements

Ideal candidates will have experience in aged care case management and have an understanding of consumer-directed care models.
